Work Environment

This position will be performed onsite at a local school, allowing for close interaction with students and staff to promote cardiovascular health education and preventative measures. Key Responsibilities

• Provide comprehensive evaluations of patients with cardiac conditions through detailed health histories and physical examinations, employing both limited and complete assessments as necessary.
• Develop and implement patient-centered treatment plans based on clinical protocols and collaborate with physicians and healthcare providers in the execution of these plans.
• Initiate appropriate diagnostic testing, including laboratory orders and imaging, assess results, and adapt treatment plans accordingly.
• Eagerly educate patients and their families about disease management strategies, lifestyle modifications, and preventative care tailored to their specific cultural and educational backgrounds.
• Continuously monitor and document patient progress, making necessary adjustments to treatment protocols based on evidence-based practices and patient feedback.
• Engage in discussions with specialists to ensure smooth transitions of care and continuity for patients needing referrals or collaborative management.
• Participate in departmental meetings to discuss new clinical practices, share insights, and remain current on advancements within cardiology.

Patient Population / Client Focus

The primary client base consists of school-aged children and adolescents, often presenting with a range of cardiovascular concerns. You will focus on both acute issues and the long-term management of chronic conditions, recognizing the importance of early intervention and education to promote lifelong health.

Qualifications

• A Master's Degree in Nursing is required, with a specialized focus on Cardiology preferred.
• Candidates must possess a valid Nurse Practitioner license in New York State.
• Demonstrated critical thinking, problem-solving, and analytical skills are essential for effective patient assessment and management.
• Excellent verbal and written communication abilities are necessary to convey complex medical information clearly and compassionately.

Preferred Experience

• At least two years of clinical experience as a Nurse Practitioner in a hospital or outpatient setting, specifically related to cardiology or a similar specialty.
