Work Environment

This role will take place predominantly in the homes of our patients throughout Rochester, New York. As a home care specialist, you'll engage with patients in a familiar environment that fosters comfort and personalized treatment. The typical work schedule is Monday through Friday, from 9 AM to 5 PM, allowing for a balanced work-life integration while also providing flexibility in your approaches to care. Our clinic utilizes the latest EMR software, specifically Web PT, to streamline clinical documentation and ensure efficient patient care management.

Key Responsibilities

• Assess and evaluate patients, developing personalized treatment plans based on their unique goals and physical capabilities.
• Administer evidence-based interventions and utilize a variety of manual therapy techniques to enhance movement and mobility.
• Maintain a manageable caseload to ensure comprehensive and focused care for each patient, allowing for the necessary time to foster effective healing relationships.
• Advocate for non-surgical treatment options, skillfully implementing cost-effective strategies that reduce reliance on medication.
• Continuously monitor patient progress and outcomes, making adjustments to treatment plans and techniques as necessary to ensure optimal recovery.
• Educate patients on exercise programs that promote strength, flexibility, coordination, and overall wellness.
• Collaborate with a multidisciplinary team of healthcare professionals, participating in regular case reviews to align on best practices and treatment goals.
• Document all patient interactions and treatment outcomes meticulously in our electronic medical records system, ensuring compliance with healthcare regulations.

Patient Population / Client Focus

Our patient population primarily consists of individuals seeking rehabilitation from orthopedic injuries, surgeries, and chronic pain conditions. We focus on providing tailored care to various demographics, including active adults, seniors, and athletes. You will have the opportunity to work closely with patients in their homes, allowing for a more personalized and intimate therapeutic experience that can significantly enhance their motivation and recovery journey.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• An active Physical Therapist license in New York State, with no history of sanctions or board proceedings.
• Demonstrated expertise in orthopedic physical therapy, with a mastery of diverse manual therapy techniques.
• Experience in developing and implementing successful, individualized exercise regimens for rehabilitation.
• Proficient in EMR software, particularly Web PT, for precise and efficient documentation.
• A commitment to continuous education and familiarization with the latest research-based physical therapy practices.

Preferred Experience

We value candidates who have a minimum of 2-3 years of experience in orthopedic outpatient settings or home health environments. Specialty certifications, such as in manual therapy or sports rehabilitation, will be considered an asset. An understanding of functional movement systems and ongoing participation in professional development activities are also highly desirable.

Compensation and Benefits

At Postural Therapeutic Outreach Clinic , we recognize the importance of our staff and their contributions. Therefore, we offer a competitive salary range of USD 100,000 to 120,000 annually, depending on experience and qualifications.
