Work Environment

This role requires the Physical Therapist to work onsite, primarily in patients' homes, where you will encounter unique challenges and diverse treatment needs. The home care setting allows PTs to observe functional impairments in their natural environment, leading to personalized treatment approaches and more meaningful therapeutic interactions. Key Responsibilities

• Provide individualized therapy sessions tailored to specific patient goals, demonstrating a unique blend of clinical knowledge and empathy.
• Maintain an optimal patient load that allows for deep engagement and attention, ensuring a thorough understanding of each patient's history and needs.
• Implement and adapt orthopedic outpatient care techniques based on evidence-informed practices and patient responses.
• Utilize innovative, cost-effective treatment methods aimed at improving range of motion, strength, and functional mobility.
• Promote non-surgical treatment modalities, minimizing reliance on medications and empowering patients to take charge of their recovery.
• Develop and execute comprehensive care plans, incorporating various therapies, exercises, and innovative modalities to enhance recovery.
• Deliver fitness and wellness regimens designed specifically to meet the diverse needs of each patient, encouraging a balanced lifestyle.
• Implement evidence-based interventions that foster long-term recovery, ensuring proper assessments and ongoing adjustments to treatment plans as necessary.
• Provide motivation and encouragement, aiding patients through their treatment journey and celebrating milestones together.
• Educate patients on the significance of maintaining strength, flexibility, balance, and coordination as integral components of a healthy lifestyle.
• Collaborate with other healthcare professionals, ensuring seamless communication and comprehensive patient care.
• Continuously assess the effectiveness of treatment strategies, documenting progress meticulously to inform future interventions.

Patient Population / Client Focus

In this role, you will work with a diverse patient demographic, including individuals recovering from surgeries, sports injuries, chronic pain, and rehabilitation needs post-accident. The ability to adapt therapy techniques to meet the unique circumstances of each patient's home environment will enhance therapeutic outcomes and patient satisfaction.

Qualifications

• Possess a completed Doctorate in Physical Therapy (DPT) from an accredited program.
• Hold an active Physical Therapist license in the state of New York, with a clean record free from sanctions or board proceedings.
• Demonstrate mastery in various manual therapy techniques, showcasing a strong foundation in therapeutic modalities.
• Ability to create and execute individualized, successful exercise programs tailored to patient abilities and recovery goals.
• Experience using EMR software, particularly WebPT, for accurate documentation and efficient patient management.
• Remain well-informed on contemporary physical therapy practices supported by current research studies.

Preferred Experience

• Prior experience in home care settings is highly desirable and will be beneficial for facilitating smooth transitions for patients.
• Familiarity with multidisciplinary teamwork approaches to enhance overall patient care.

Compensation and Benefits

We offer an attractive annual salary range of USD 90,000 to 110,000, which may vary based on experience and expertise.
