Work Environment

The work environment is centered around home care, providing an intimate and supportive atmosphere for both the therapist and their clients. You will be expected to make home visits to evaluate and treat patients within their home settings, which can often provide unique insights into their rehabilitation needs. Key Responsibilities

• Direct and manage physical therapy treatment plans focused on alleviating pain, preventing disabilities, and maximizing specific functional improvements tailored to each client's capabilities.
• Conduct thorough evaluations of clients' physical function through diagnostic testing, followed by observing and documenting their response to therapies in real-time.
• Evaluate each patient's home environment to design personalized recommendations aimed at enhancing independence and safety during rehabilitation.
• Oversee Physical Therapy Assistants to ensure the effective and appropriate delivery of agreed-upon care plans.
• Administer therapeutic exercises and treatments, including manual therapies, traction, and the application of physical agents, while providing motivation and guidance to clients.
• Engage actively in the interdisciplinary team, participating in case conferences to update and refine treatment plans based on client progress.
• Document patient evaluations, treatments, and progress notes consistently to maintain robust clinical records and ensure compliance with healthcare regulations.
• Instruct and supervise Physical Therapy Assistants in the application of therapy techniques, ensuring the highest quality of care is offered to all patients.

Patient Population / Client Focus

The patients you will work with are diverse, ranging from those recovering from surgery or injury to individuals with chronic conditions requiring ongoing rehabilitation. The focus is on empowering clients of all ages, from young adults to seniors, in regaining and maintaining their independence and enhancing their overall health and well-being.

Qualifications

• A Bachelor's or Doctorate degree in Physical Therapy from an accredited program.
• Current and valid physical therapy licensure in the state of New York.
• Current CPR certification and valid driver's license with automobile insurance.
• A demonstrated commitment to clinical excellence, quality, and integrity, with exceptional interpersonal communication skills.
• Familiarity with federal and state regulations governing physical therapy practices.

Preferred Experience

• At least one year of clinical experience in acute care or home care settings.
• Experience with electronic health record systems and documentation relevant to patient care.
• A proven track record of collaboration within multidisciplinary healthcare teams.
• Skills in advocacy for patient care within home healthcare and related service lines.

Compensation and Benefits

This role offers a competitive salary range of USD 90,000 – 100,000 annually, commensurate with experience.
