Work Environment

This full-time position is based onsite, emphasizing face-to-face interaction and relationship-building with patients. The work setting includes providing services through concierge medicine and house calls, allowing you to meet patients in the comfort of their homes or at our dedicated facility. Key Responsibilities

• Conduct comprehensive assessments and diagnostic evaluations for both acute and chronic cardiovascular conditions, applying evidence-based practices to form accurate diagnoses.
• Develop individualized treatment plans that encompass medication management, lifestyle modifications, and ongoing surveillance of patients' health statuses.
• Facilitate collaborations with local community organizations to design and implement innovative health education programs that inform and engage patients.
• Provide culturally competent care, addressing barriers to access and advocating for patients’ rights and healthcare needs.
• Ensure continuity of care by coordinating effective referrals and follow-up appointments, keeping meticulous records of patient interactions and care plans in our electronic health record (EHR) system.
• Participate actively in community events, health fairs, and outreach initiatives to promote heart health awareness and provide essential screenings.

Patient Population / Client Focus

Your role will focus on diverse populations, particularly underserved communities, emphasizing the importance of accessible cardiovascular care. By building trust and understanding each patient's unique background, you will enhance their health literacy and encourage proactive health management.

Qualifications

• Possess a current certification as a Nurse Practitioner with a focus in Cardiology or related fields.
• Hold an active state nursing license to practice in New York.
• A Master’s degree in nursing from an accredited institution is mandatory.

Preferred Experience

• A minimum of two years (with a preference for up to five years) of clinical experience in community health or public health settings, particularly focused on cardiovascular health.
• Experience working within a concierge medicine model and familiarity with technology-based solutions in healthcare delivery.

Compensation and Benefits
