Key Responsibilities Deliver comprehensive ongoing care for individuals and families at all ages, from children to seniors. Administer preventive and acute care, manage chronic diseases, and address mental health issues utilizing a patient-centric philosophy. Perform physical examinations, interpret tests, and formulate tailored treatment plans. Collaborate closely with care coordinators and specialists to provide integrated holistic health. Engage in quality improvement projects that advance patient care outcomes and operational performance. Maintain accurate patient documentation through electronic medical records (EMR). Offer both in-person and telehealth services to meet diverse patient needs. Education & Qualifications MD or DO degree from an accredited medical institution. Completion of an accredited Family Medicine residency program. Certifications Board Certified in Family Medicine. Eligible for (or currently hold) an active Colorado medical license. BLS and ACLS certifications are preferred. Attributes & Experience Expertise in diagnostics and patient care emphasizing preventive measures and proactive treatment. Comfort with value-based care methodologies and a drive for continuous improvement. Exceptional communication and people skills with a collaborative mindset. Proficient in the latest technology, including EMRs and telemedicine applications. A passionate commitment to providing inclusive care to diverse patient populations.
