Family Case Manager

This is not your typical case manager position. It is much more. At Family Promise of Bergen County, we meet families where they are; we don't just leave them there. Family Success Strategists , as we call case managers, empower families to understand their strengths and challenges and together explore ways to move forward toward a more secure future. We have the responsibility to think creatively and provide holistic support while making sure families maintain agency and dignity. Responsibilities: Perform initial screening and determine eligibility Employ critical thinking to determine ways to assist each family Assist families in identifying their skills, strengths, and interests Assist families in setting goals/identifying achievable steps Develop a budget and enroll each family into savings plan Assist in career exploration Work together with family to identify skill sets necessary to do these jobs Review progress/outcomes with Director of Family Services and Executive Director Perform exit interviews and provide follow-up resource referrals Keep accurate progress notes Responsible for HMIS data input and program statistics Research training programs for jobs that pay a living wage Attend meetings as determined by the Director of Family Services Requirements: Adult case management experience is essential Bachelor's Degree in related field or 5+ years of case management experience Excellent written and verbal communication skills Knowledge of basic financial literacy concepts Knowledge of social service system Hours vary but include some evening hours Must have reliable car Cover letter and resume required to be considered Knowledge of HMIS 401(k) 401(k) matching Health insurance Paid time off Vision insurance Salary: $55,000.00 - $60,000.00 per year Work Location: In-person #J-18808-Ljbffr Family Promise of Bergen County
