Work Environment\n

In this remote position, our Licensed Clinical Social Worker will primarily liaise with families of young children facing developmental delays or challenges. We employ a collaborative and compassionate approach, ensuring our team members are well-supported—offering resources and continuous development opportunities to enhance their skills. The role will primarily interact with families and children remotely while occasionally engaging with interdisciplinary teams at our Hospice Center.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the needs of children and families.

• Develop and actively participate in Individualized Family Service Plan (IFSP) creation and implementation.

• Monitor and document outcomes, working closely with a multidisciplinary team to evaluate the effectiveness of interventions.

• Establish rapport with children and their families, fostering a nurturing environment conducive to learning and growth.

• Provide coaching and personalized guidance in alignment with evidence-based practices and Natural Learning Environment methodologies.
\n
• Explore and recommend strategies that enhance child development while addressing family concerns and priorities.

• Comply with federal and state regulations to ensure a safe and responsive service delivery environment.

• Facilitate resource access for families to support their child's developmental journey.

Patient Population / Client Focus\n

The primary focus of this role is to support infants and toddlers experiencing delays or disabilities that affect their physical, cognitive, social-emotional, and communication skills. The LCSW will work closely with families, employing individualized strategies to foster an environment of growth and encouragement, ultimately promoting the health and development of their children.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Current LCSW licensure issued by the Department of Public Health.
