Key Responsibilities\n

• Plan and coordinate individualized treatment packages that address the unique needs of clients dealing with mental health issues or terminal illnesses.

• Conduct in-depth interviews with clients to assess their current mental, emotional, and social condition, identifying their specific strengths, weaknesses, and needs.

• Establish measurable, tailored goals for each case, ensuring they align with the clients’ desires and foster a sense of achievement.

• Monitor client progress through regular assessments and adapt treatment methods when necessary, ensuring that care remains relevant and effective.

• Provide evidence-based counseling sessions, offering information and guidance on strategies for overcoming challenges and enhancing well-being.

• Maintain meticulous records of client interactions and progress, ensuring compliance with legal standards and organizational policies.

• Participate in case conferences, sharing insights and working collaboratively to develop comprehensive care approaches.

• Serve as the primary support contact for assigned clients, collaborating with community services to facilitate ongoing recovery and support.

This role primarily serves a diverse population of clients including individuals experiencing mental health crises, those managing chronic psychological conditions, and patients in hospice care.
