Key Responsibilities\n

• Conduct thorough psychosocial assessments using interviews, observations, and the examination of medical records to identify emotional, social, and environmental factors affecting patient recovery.

• Work closely with the rehabilitation team to develop personalized treatment plans that include psychosocial considerations alongside physical rehabilitation goals.

• Help clients navigate community resources by providing information on support groups, vocational training, and assistance in housing or financial support.

• Facilitate educational sessions for patients and families about community resources and post-rehabilitation services, preparing them for successful transitions.

• Engage in discharge planning efforts to ensure smooth transitions from our detox center to home or other necessary care settings, fostering sustained recovery.

Patient Population / Client Focus\n

At the detox center, our patient population includes individuals grappling with substance use disorders, those recovering from physical injuries, and individuals facing significant psychological challenges. The LCSW will provide integral support through emotional and social challenges faced during recovery, aiming to empower patients as they transition toward independence and reintegration into their communities.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
