Occupational Therapist (OT) – Medicare Home Health Location: Sunny Isles Beach, Bal Harbour, Surfside, and Miami Beach. Employment Type: Part-Time / PRN Schedule: Flexible, field-based Reports To: Clinical Manager / Director of Clinical Services Responsibilities Perform comprehensive occupational therapy evaluations, including SOC, Recertification, and Discharge assessments. Develop and implement individualized treatment plans focused on functional independence and safety. Provide interventions including ADL/IADL training, upper extremity strengthening, fine motor coordination, cognitive strategies, and adaptive equipment recommendations. Educate patients and caregivers on home safety, energy conservation, and compensatory techniques. Accurately complete all documentation in the EMR within required timeframes. Collaborate with physicians, nurses, therapists, and interdisciplinary team members. Participate in case conferences and care coordination as needed. Maintain compliance with Medicare, ACHC, and agency standards. Qualifications Current FL licensure as an Occupational Therapist (OT). Minimum 1 year of clinical experience; home health experience preferred. Familiarity with WellSky EMR or similar documentation systems preferred. Strong clinical reasoning, time management, and communication skills. Ability to work independently and manage a flexible field-based schedule.
