Key Responsibilities\n

• Provide direct clinical services to patients suffering from mental illnesses and severe emotional issues, ensuring their social, mental, and emotional needs are met.

• Conduct detailed assessments and screenings to establish appropriate care plans, utilizing evidence-based practices during evaluations.

• Engage in multidisciplinary team meetings to coordinate care strategies, ensuring comprehensive support tailored to each patient.

• Develop and implement integrated care plans, collaborating with family members and other healthcare providers to promote continuity of care.

• Monitor and document patient progress, crisis intervention efforts, and treatment modifications, ensuring adherence to ethical guidelines and organizational protocols.

• Carry an active caseload, providing individual, group, and family therapy sessions to facilitate healing and coping strategies for clients and their families.

• Identify and address any unsafe situations, prioritizing the safety and well-being of all clients and staff.

Patient Population / Client Focus\n

The LCSW will primarily serve clients within a Hospice Center, a unique patient population characterized by individuals experiencing life-limiting illnesses. Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 85,000 annually, reflecting experience and licensure.
