Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the psychosocial needs of seniors under hospice care.

• Develop and implement Individual Family Service Plans (IFSPs) in collaboration with clients, families, and interdisciplinary team members.

• Monitor client outcomes and provide feedback to the team to ensure effective interventions and care coordination.

• Offer emotional support and essential resources to families navigating grief and loss, facilitating a safe space for open discussions regarding their loved one's care.

• Adhere to all Federal and State regulations to maintain a high standard of ethical and professional practice.

• Build rapport with clients, families, and team members, promoting a healthy and engaging environment conducive to open communication.

• Utilize evidence-based practices to guide treatment plans and adjust services as necessary to meet the evolving needs of clients.

Patient Population / Client Focus\n

This position focuses on serving seniors enrolled in hospice care, along with their families. You will address concerns that arise during this phase of life, such as end-of-life issues, grief counseling, emotional support, and resource connection. Your direct involvement will significantly impact their emotional and mental well-being, allowing them to navigate these sensitive times with dignity and support.

Qualifications\n

• Master’s degree in Social Work (MSW) from an accredited institution.

• Current and active LCSW licensure in the state of Texas.
