Full-Time Physical Therapist - Outpatient Assisted Living Care

Work Environment

The Physical Therapist will work onsite in our outpatient office , where we foster a warm, inviting, and collaborative atmosphere. Interacting closely with residents allows you to build meaningful relationships while implementing your expertise in physical rehabilitation. Key Responsibilities

• Conduct thorough evaluations for new and returning residents to assess their mobility, strength, and overall functional capabilities.
• Create and implement individualized treatment plans based on residents' unique goals, preferences, and therapeutic needs.
• Administer one-on-one therapy sessions either in residents' apartments or designated onsite therapy areas, focusing on enhancing mobility and reducing pain.
• Maintain accurate documentation of patient progress in the electronic medical records (EMR) system, ensuring compliance with healthcare regulations.
• Work collaboratively with nurses, caregivers, and rehabilitation staff to design and execute effective fall prevention and wellness programs.
• Educate residents and their families about safe mobility strategies, appropriate use of assistive devices, and at-home exercises to promote recovery and independence.

Patient Population / Client Focus

This position primarily involves working with older adults residing in an assisted living community. Residents may face a variety of challenges linked to age, including but not limited to mobility restrictions, chronic pain, and the need for rehabilitation following surgery or injury. Your role is pivotal in not only addressing their physical needs but also in uplifting their spirits and enhancing their daily activities through targeted therapeutic interventions.

Qualifications

• A current and valid New Jersey Physical Therapist License .
• Completion of an accredited Physical Therapy program (DPT, MPT, or equivalent).
• Hold a current CPR/BLS certification.
• Excellent interpersonal and communication skills, essential for building rapport with both residents and staff.
• A deep-seated passion for working with older adults and understanding their unique healthcare needs.

Preferred Experience

• A minimum of 2 years experience in a physical therapy setting, preferably with a focus on geriatric care or assisted living.
• Familiarity with EMR systems and documentation protocols.

Compensation and Benefits
