Key Responsibilities\n

• Conduct thorough psychosocial assessments through various means, including patient interviews, observations, and detailed medical record reviews. This information will be instrumental in understanding the emotional and environmental factors affecting each patient’s recovery.

• Offer professional counseling to patients and their families, providing essential support during emotional adjustments and struggles that may arise during the rehabilitation process.

• Collaborate closely with the interdisciplinary rehabilitation team to create and implement individualized treatment plans that holistically address each patient's psychosocial needs.

• Guide patients in navigating and accessing crucial community resources, such as vocational rehabilitation, support groups, and housing assistance, to reinforce their support system during recovery.

• Educate patients and their families on available community resources and post-rehabilitation services, ensuring a smooth transition after discharge.

• Participate actively in discharge planning processes, facilitating the transition to home or alternative care settings while ensuring that ongoing support is in place.
