Key Responsibilities\n

In your capacity as an LCSW, you will be responsible for a range of critical functions, including:

• Conducting thorough assessments to determine the counseling needs of clients in relation to their mental health and substance abuse challenges.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria to diagnose clients for substance use disorders and collaborating to determine suitable levels of care according to ASAM (American Society of Addiction Medicine) standards.
\n
• Developing tailored treatment plans that address the specific substance abuse counseling needs of each client.
\n
• Creating comprehensive discharge and continuum of care plans to facilitate transitions to suitable follow-up services.
\n
• Maintaining meticulous documentation of all counseling services, assessments, and client interactions within the clinical records.
\n
• Managing a diverse caseload while ensuring regular case management and utilization reviews are conducted.
\n
• Coordinating additional support services for clients and their families, documenting all progress and services in client records effectively.
\n
• Engaging with referral sources, including legal, educational, and workplace entities, to provide holistic support.

• Facilitating interventions for crisis situations through a multi-disciplinary team approach to ensure immediate and effective care for clients.
