Key Responsibilities\n

• Provide individual psychotherapy and counseling services to hospice patients facing mental health challenges associated with their diagnoses.

• Conduct comprehensive assessments of patients’ emotional, behavioral, and social functioning to inform treatment planning.
\n
• Develop and implement personalized treatment plans that are culturally sensitive and utilize evidence-based practices catering to both the individual’s needs and their family's dynamics.

• Facilitate supportive group therapy sessions for families, helping them cope with the behaviors and emotions that arise during the end-of-life process.

• Collaborate effectively with interdisciplinary teams, including doctors, nurses, and chaplains, to create a holistic support system for patients, ensuring an integrated approach to care.

• Maintain accurate and timely electronic health records (EHRs), documenting progress and treatment outcomes to support clinical decision-making.
\n
• Engage in continuous professional development through training, supervision, and staying updated with the latest mental health trends and treatment strategies.

Patient Population / Client Focus\n

In this role, you will work primarily with patients experiencing terminal illness and the spectrum of mental health conditions that accompany such diagnoses, including anxiety, depression, and PTSD. Your empathetic approach will help ease the emotional distress that often impacts patients and their families, providing them with the necessary support to navigate profound life changes.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• Exceptional communication and interpersonal skills, enabling effective relationships with patients, families, and healthcare colleagues.

Compensation and Benefits\n
