Physical Therapist (PT) - Home Health

Home Health Physical Therapist

8-hour days only; no weekends or on-call duties.

May visit up to 6 patients per day, with a standard of 30 productivity points.

1 year of skilled home health experience required.

BCLS certification required.

Experience with geriatrics, home health, OASIS documentation, and orthopedics preferred.

Provide direct physical therapy services to patients in their home settings.

Assess patient needs and develop tailored treatment plans.

Maintain accurate and up-to-date patient documentation using OASIS guidelines.

Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Educate patients and their families on treatment plans and home exercises.

Orientation is 16 hours; background and reference checks required. Please confirm any restrictions on time-off requests during the application process.
