- Conduct comprehensive evaluations for patients in both inpatient and outpatient scenarios and develop tailored treatment plans.
- Implement physical therapy interventions targeting mobility, strength, balance, and overall functional improvement.
- Work alongside nurses, physicians, case managers, and other rehabilitation specialists to ensure optimal patient outcomes.
- Guide patients and their families through therapeutic objectives, discharge processes, and at-home exercise routines.
- Maintain detailed documentation of evaluations, treatments, and progress in alignment with facility and regulatory protocols.
- Engage in collaborative care team rounds and interdisciplinary meetings to foster coordinated efforts.
- Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution.
- Valid state licensure as a Physical Therapist
- Preferred: CPR certification
- Specialized certifications (such as neurological or geriatric) are valued but not essential.
- Prior experience in a similar role is highly desirable.
- Ability to effectively treat patients with intricate medical conditions and mobility challenges.
- Excellent communication and team-driven skills.
- Proficiency with digital charting and clinical software.
