The Charles Lea Center is seeking a detail-oriented Incident Management & Risk Coordinator to oversee incident reporting, investigations, and risk management activities. This role ensures compliance with OIDD requirements, including Abuse, Neglect, and Exploitation (ANE) reporting, while analyzing incident trends, monitoring corrective actions, and supporting quality improvement initiatives. Working closely with leadership, program staff, and stakeholders, the coordinator helps protect the health, safety, rights, and well-being of individuals supported while promoting regulatory compliance and service excellence.
Salary Range
$70,000 - $80,00
Position Summary
The Incident Management (IM) Director has overall responsibility for the incident management process at CLC which includes allegations of Abuse, Neglect and/or Exploitation (ANE) and other types of sentinel events and/or Critical Incidents, which may affect the health, safety, or welfare of the individuals supported. The IM Director maintains the overall responsibility to ensure completion of the incident reporting and investigation process in compliance with all applicable laws, regulations, policies and procedures.
Key Responsibilities
Incident Management & Compliance
- Maintain overall responsibility for the agency's incident management process in accordance with OIDD regulations and requirements.
- Ensure timely reporting, investigation, documentation, and follow-up of all reportable incidents, including allegations of Abuse, Neglect, and Exploitation (ANE), sentinel events, and critical incidents.
- Take appropriate and timely action in response to incidents involving the health, safety, welfare, and rights of individuals supported.
- Ensure all incidents meeting OIDD reporting criteria are properly submitted and documented.
- Develop and maintain organizational policies, procedures, and structures that support incident identification, reporting, and resolution.
- Conduct and/or oversee administrative reviews and ensure reports meet quality and compliance standards prior to submission to OIDD.
- Maintain accurate records, reports, and files related to investigations, incident reviews, and corrective actions.
Risk Management & Quality Improvement
- Gather, monitor, and analyze agency incident data on an ongoing basis.
- Review incident trends, medication errors, restraints, corrective action plans, and other risk indicators to identify opportunities for improvement.
- Conduct monthly monitoring and quarterly trend analyses of incident data.
- Present findings and recommendations to agency leadership and the Risk Management Committee.
- Identify and implement individual and systemic improvements based on quality assurance and risk management findings.
- Support agency-wide efforts to prevent incidents and reduce organizational risk.
Collaboration & Stakeholder Engagement
- Collaborate with program staff, leadership, families, advocates, and external stakeholders to address incidents and reduce risk.
- Coordinate meetings and follow-up activities related to abuse, neglect, exploitation, and critical incidents.
- Ensure individuals supported and their families receive information regarding incident reporting processes and rights protections in accessible formats that meet their communication needs.
- Serve as a resource and subject matter expert regarding ANE reporting and incident management requirements for staff in South Carolina, North Carolina, and Tennessee.
Training & Staff Development
- Assist with the development and delivery of staff training related to:
- Abuse, Neglect, and Exploitation (ANE)
- Individual rights
- Incident reporting requirements
- Risk management and quality assurance practices
- Promote a culture of dignity, respect, accountability, and compliance throughout the organization.
Complaint Resolution
- Ensure processes are in place to receive, document, investigate, and resolve complaints involving health, safety, rights, and quality-of-care concerns.
- Safeguard against retaliation, intimidation, or threats related to the reporting of complaints or allegations.
- Monitor complaint resolution activities and recommend improvements as needed.
Education & Experience
Education
- Bachelor's degree in Human Services, Social Work, Psychology, Special Education, Healthcare Administration, Nursing, or a closely related field required.
- Master's degree preferred.
Experience
- Minimum of three (3) years of experience working with individuals with intellectual and developmental disabilities or other related populations.
- Experience conducting investigations, quality assurance activities, incident management, compliance oversight, or risk management preferred.
- Supervisory experience preferred.
Work Environment
This position operates primarily in a professional office environment. Regular interaction with staff, leadership, and cross-functional teams is required.
Physical Requirements
This role involves regular standing, walking, lifting (up to 15 lbs), and hands-on support. Reasonable accommodations are available.
