Work Environment

This role requires an onsite presence, as therapy is delivered in the homes of our patients, many of whom may face mobility challenges. As a valued member of our home health team, you will be empowered to develop personalized, therapeutic treatment plans. Your capability to adapt to various home settings while ensuring patient safety and comfort is paramount to this role.

Key Responsibilities

• Develop and implement individualized treatment plans tailored to each patient's specific needs, with a focus on improving mobility, strength, and overall functionality.
• Engage directly with patients, guiding them through therapeutic exercises and activities designed to alleviate pain and promote physical well-being.
• Educate patients and their families on effective body mechanics and exercise techniques, providing them with home exercise programs essential for their recovery.
• Administer various modalities such as electrical stimulation, ultrasound, and heat/cold therapy, under your professional guidance and supervision.
• Monitor and document patient progress meticulously, tracking range of motion, strength gains, and improvements in daily functioning.
• Ensure treatment areas are prepared and well-equipped, managing supplies and equipment to optimize therapy sessions.
• Collaborate with a multidisciplinary team, including Occupational Therapists and Speech-Language Pathologists, to enhance patient care and outcomes.
• Provide emotional support to patients, fostering a positive therapeutic environment that encourages motivation and recovery.
• Adhere to ethical and professional standards, emphasizing confidentiality and respect for patients' rights throughout therapy.

Patient Population / Client Focus

The ideal candidate will have the opportunity to work with a diverse patient population, including individuals recovering from surgeries, sports injuries, and neurological conditions. You will engage closely with elderly patients facing age-related limitations, as well as children requiring specialized rehabilitation services. Your work not only addresses their physical rehabilitation goals but also significantly enhances their overall quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Active state license to practice as a Physical Therapist in New Jersey.
• Minimum of one year of experience in physical therapy, preferably in a home care or outpatient setting.
• Strong communication skills and an empathetic approach toward patient care are essential.
• Ability to work independently and maintain effective communication with team members.

Preferred Experience

• Experience in home care settings with specific populations, including geriatrics and pediatrics.
• Proficiency in developing detailed and effective treatment plans.
• Familiarity with electronic health record (EHR) systems and clinical documentation processes.

Compensation and Benefits
