Position Overview

The Premier Health and Wellness Center is actively seeking a dedicated and skilled Physical Therapist (PT) to join our team in Port Elizabeth, New Jersey. Work Environment

This role is based onsite in our home care facility, where you will work closely with a team of skilled professionals, including other Physical Therapists, Occupational Therapists, and Speech-Language Pathologists. Our clinic operates from Monday to Friday, 8:00 am to 5:00 pm, with occasional flexibility required to accommodate various patient needs and the dynamic nature of our operations. You will find our work environment to be collaborative and team-oriented, with a shared goal of delivering the highest quality of care.

Key Responsibilities

• Design and implement individualized treatment plans that cater to each patient's specific rehabilitation needs, focusing on improving their mobility and strength.
• Guide patients through therapeutic exercises, stretches, and activities, ensuring they understand proper techniques and are motivated throughout their recovery journey.
• Educate patients and their families on appropriate body mechanics, exercise regimens, and home exercise programs to promote recovery and prevent further injury.
• Administer treatment modalities such as electrical stimulation, ultrasound, and heat/cold therapy to assist in pain relief and recovery.
• Monitor and document patient progress meticulously, tracking improvements in range of motion, strength, and overall functionality.
• Prepare and maintain treatment areas and ensure that all equipment and supplies are well organized and ready for use during therapy sessions.
• Collaborate with a multidisciplinary team to optimize patient care through effective communication and partnership in treatment planning.
• Provide emotional support to patients, helping them stay positive and motivated during challenging rehabilitation phases.
• Uphold ethical and professional standards by maintaining patient confidentiality and respecting the rights and privacy of patients.

Patient Population / Client Focus

At Premier Health and Wellness Center, you will work with a diverse patient population comprising individuals recovering from surgeries, injuries, disabilities, and chronic conditions. Your role will range from assisting young athletes with sports-related injuries to helping elderly patients regain independence after major surgeries. Each interaction will deepen your understanding of patient needs and enhance your skills in adaptive therapy techniques.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid state license to practice as a Physical Therapist.
• A minimum of one year of relevant clinical experience is preferred, demonstrating your ability to effectively assess and treat a variety of conditions.

Preferred Experience

• Experience in a home care setting or with specific patient populations mentioned above.
• Familiarity with electronic health record (EHR) systems and documentation best practices.
• Additional certifications in specialized rehabilitation areas (orthopedic, geriatric, pediatric, etc.) are a plus.

Compensation and Benefits
