Agile Stride Rehab, a dedicated provider of outpatient therapy services, is seeking full-time Physical Therapists to join our team in Port Elizabeth, New Jersey. Work Environment

This position is located onsite, where you will deliver home care therapy services directly to our geriatric clients in their residences. You will work Monday through Friday, from 9 am to 5 pm, allowing for a structured schedule that supports work-life balance. Key Responsibilities

• Perform comprehensive evaluations of patients' physical capabilities, documenting findings and developing individualized treatment plans thoughtfully tailored to their specific goals.
• Administer various therapeutic interventions—such as therapeutic exercises, manual therapy techniques, and modalities—to enhance patient recovery and mobility.
• Monitor, document, and analyze patient progress meticulously, adapting treatment strategies as necessary to ensure that each patient achieves the best possible outcomes.
• Collaborate effectively with a multidisciplinary healthcare team that includes physicians, occupational therapists, and speech therapists, creating a holistic approach to each patient's care plan.
• Educate patients and their families about therapeutic techniques, suggested home exercises, and approaches for injury prevention, empowering them in their recovery.
• Implement strategies for fall prevention and safety within the clients' homes, significantly reducing the risk of accidents and promoting independence.
• Engage in ongoing education and professional development to remain current with advancements in physical therapy practices and enhance skill sets.
• Maintain high ethical and professional standards, ensuring confidentiality and a commitment to the highest quality of patient care.

Patient Population / Client Focus

Your primary focus will be our geriatric clientele, a group that often faces unique health challenges. Many of our patients experience mobility issues, chronic pain, and other age-related conditions. Providing physical therapy in the comfort of their homes not only improves their physical wellbeing but also significantly contributes to their emotional and mental health by allowing them to stay within familiar surroundings while receiving care.

Qualifications

• Must possess a Bachelor's Degree in Physical Therapy from an accredited college or university.
• Must hold an active state licensure to practice as a Physical Therapist.
• Strong communication and interpersonal skills are essential for effectively engaging with patients, their families, and other healthcare professionals.
• Proficiency in electronic medical records (EMRs) and documentation tools is necessary for accurate patient tracking and reporting.
• A solid understanding of evidence-based practices and a willingness to apply the latest therapeutic techniques is crucial.

Preferred Experience

Compensation and Benefits

In recognition of your expertise and commitment, the annual salary for this position ranges from $100,000 to $120,000, commensurate with experience.
