Join a dynamic healthcare organization in the Pacific Northwest region as an Administrator overseeing Population Health Programs. This leadership role is pivotal in advancing organizational goals through the development and management of population health initiatives, payor contract strategies, and data-driven performance management.
Job Summary: Lead efforts to optimize value-based care initiatives by collaborating with clinical, operational, and external partners. Drive improvements in quality, cost reduction, and patient experience through innovative program design and evaluation.
Key Responsibilities
- Lead population health and value-based care initiatives by partnering with clinical and operational leaders to leverage strategies and data analytics that improve outcomes and reduce costs.
- Manage payor strategy and contract performance, aligning agreements to maximize shared savings and organizational priorities.
- Oversee program evaluation and innovation, including pilot designs, proformas, and ROI analyses to support scalable initiatives.
- Collaborate with internal and external stakeholders such as providers, payors, and community partners to advance population health objectives.
- Manage daily departmental operations, address challenges, and resolve personnel issues.
- Ensure strong financial performance through trend analysis and forecasting to inform strategic growth.
- Develop frameworks for data governance, key performance indicators, and actionable reporting to support decision-making.
- Lead special projects and other duties as assigned.
Leadership and Management
- Build and maintain effective relationships with team members and peers, fostering collaboration and alignment.
- Communicate a compelling vision and motivate staff to advance organizational goals.
- Manage financial resources, staffing, and departmental performance to meet service expectations.
- Ensure compliance with regulatory requirements and maintain accurate records.
- Promote professional development and provide clear feedback to support employee growth.
- Drive results by aligning team efforts, monitoring progress, and delivering measurable outcomes.
- Foster an environment of trust, integrity, and respect.
- Lead innovation by applying research and creativity to departmental improvements.
- Manage change effectively, adapting workflows and engaging staff in transitions.
- Participate actively in strategic planning and develop departmental strategies to address future challenges.
Education and Experience
- Master's degree or equivalent required; BSN preferred.
- Minimum of two years supervisory experience.
- At least two years of clinical experience related to population health, value-based contracting, and care coordination/management.
Licenses and Certifications
- Basic Life Support (BLS) certification required within 60 days of hire.
Knowledge, Skills, and Abilities
- Strong understanding of population health practices and value-based care models.
- Excellent leadership, facilitation, delegation, and coaching skills.
- Proficiency with Microsoft Office and electronic medical records systems.
- Effective written and verbal communication skills tailored to diverse audiences.
- Ability to maintain accurate documentation and implement process improvements.
- Strong problem-solving, organizational, and prioritization abilities.
- Capacity to work independently and exercise sound judgment.
