Social Worker/Case Manager

Benefits: Health insurance Paid time off About the Role: SCLAD (Spinal Cord Living-Assistance Development, Inc.) is a community-based organization that serves persons with physical disabilities to promote independent living. SCLAD is looking for a professional Social Worker/Case Manager (CM) to join its team in Hialeah. In this challenging role, the CM will make a significant impact in the lives of Program Participants (PP) by connecting them with the resources and support they need to thrive; and to the greater community in which they live. This is a very challenging and rewarding position. Responsibilities: In-depth intake interviews, basic social assessments to ascertain specific needs Development of an Individual Service Strategy (ISS) based on intake data and assessment Implement ISS, in conjunction with PP, by identifying and connecting PP with resources in the community that respond to the ISS. Develop, with PP, a reliable support network that may include family, friends, healthcare providers, therapists and other professionals Monitor PPs’ continuum of care; quality of support and services that form part of support network Document and report on PP’s progress; updating ISS as needed. Advocate on behalf of PM to promote access to services and successful rendition of same Requirements: Bachelor's (or Master's) degree in Social Work (BSW/MSW) or related field Prior experience in case management, disability services, or rehabilitation settings a plus Strong knowledge of community resources, Medicaid/Medicare, and social service systems in Florida Excellent communication, organizational, and problem-solving skills Bilingual (English/Spanish) preferred given the Hialeah, FL community Empathetic, patient, and client-centered approach to care Interested/integrated in social media Technologically inclined Interested in learning new/related fields About Us: Spinal Cord Living-Assistance Development, Inc.
