Description
The hiring range for this position is $86,064 - $96,252. The full range for this position is $86,064 - $120,391. The Care Coordination Supervisor is responsible for managing the county's Care Coordination team, which implements managed care organization care coordination and Model of Care (MOC) requirements for members enrolled in Minnesota Senior Health Options (MSHO), Minnesota Senior Care Plus (MSC+), and Special Needs BasicCare (SNBC). This position provides leadership, supervision, training, and quality assurance oversight to ensure care coordination services meet state and federal regulatory and quality requirements, standards and compliance with the county's delegation agreement with any managed care organization. This position will also include oversight and supervision of Elderly Waiver for Fee for Service and Alternative Care clients. The Care Coordination Supervisor reports directly to the Health and Human Services (HHS) Director and serves as an active member of the HHS Management Team.

Job Duties
- Serve as an active member of the HHS Management Team, working collaboratively with the HHS Director and other supervisors on departmental policies, programs, services, finance and budgeting, performance indicators and evaluation, and personnel management.
- Provide agency leadership in the absence of the HHS Director, in partnership with the HHS Management Team, to address immediate agency needs and emergency response situations.
- Provide fiscal oversight and budgetary responsibility for the Care Coordination team, including monitoring staff billing practices and service documentation to ensure accuracy, compliance, and maximization of reimbursement within the health plan.
- Provide leadership, supervision, training, and quality assurance oversight for Care Coordinators to ensure compliance with Kandiyohi County managed care organization delegation agreement care coordination model and applicable DHS, CMS, and NCQA requirements.
- Collaborate with MCO care management staff, providers, and other identified individuals as part of a multi-disciplinary team to coordinate member care plans that ensure the appropriateness, effectiveness, quality, and cost-effectiveness of services provided to members.
- Oversee implementation and monitoring of care coordination roles and responsibilities, including fast-track interventions, customized living processes, and the residential services tool, as applicable.
- Ensure timely and accurate completion of care plans, health risk assessments, and required MOC documentation in accordance with established timelines.
- Provide oversight to ensure compliance with the county's delegation agreement, including monitoring care plan activities and case management ratios and outcomes for MSHO, MSC+, and SNBC populations.
- Facilitate communication and coordination between Kandiyohi County and MCO regarding care coordination policy, program, and technical issues.
- Participate in the development, implementation, and monitoring of continuous care coordination and quality improvement plans.
- Conduct or coordinate care coordination education and training sessions on the care coordination model, MCO requirements, and expected outcomes.
- Monitor utilization management activity pertinent to assigned areas of responsibility and report findings as required.
- Provide leadership in personnel matters, including hiring, orientation, training, education, promotion, discipline, and discharge.
- Assess staff workloads and make appropriate assignments or adjustments to support effective service delivery.
- Participate in staff performance evaluations according to established Human Resources procedures to promote high-quality services and encourage employee development.
- Participate in community, regional, and statewide initiatives related to the MSHO, MSC+, and SNBC populations, including meetings with partners as they relate to care coordination activities.
- Represent the agency in professional organizations, community activities, meetings, and collaborative initiatives related to care coordination.
- Perform other duties as assigned of a similar nature or level.
- Bachelor's degree or higher from an accredited college or university in Public Health, Nursing, Social Work, or another closely related field.
- A minimum of three (3) years of prior supervisory experience in human services and/or public health.
- Extensive knowledge of case management and care coordination principles and practices.
- Experience in care coordination, case management, or managed care program administration.
- Experience or demonstrated ability in staff supervision and leadership.
- Current or applicable knowledge of managed care programs, including MSHO, MSC+, and SNBC.
- PrimeWest Health's care coordination model, Model of Care (MOC) requirements, and delegation agreement expectations, preferred not required.
- DHS, CMS, and NCQA structure, process rules, and reporting requirements applicable to managed care and Special Needs Plans (SNP).
- Principles and practices of case management and care coordination.
- Waiver and managed care programs and their relationship to county-administered services.
- Community resources and services available to individuals receiving long-term services and supports.
- Community health resources and social service agencies.
- Principles and practices of staff supervision, training, and quality assurance.
- Personnel management practices, including hiring, orientation, training, performance management, discipline, and discharge.
- Quality improvement processes and data interpretation.
- Time management, scheduling, workload management, and independent work planning.

- Monitoring quality assurance and ensuring compliance with applicable policies, programs, and procedural requirements, including county's health plan delegation agreement.

- Networking and coordinating effectively with community resources, health plans, and DHS personnel.
- Managing multiple priorities, deadlines, and program requirements.

- Ability to sit or stand for extended periods for meetings, care plan reviews, and documentation.
- Ability to occasionally lift and carry materials or files up to 25 pounds.
- Ability to travel for provider meetings, professional meetings, and training, including occasional overnight travel.
