Hospice RN Case Manager

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy

Requirements include an active RN license in the state of employment, graduate of an accredited school of Nursing, one (1) + year hospice, home health or related organization experience preferred, compassion for patients and families navigating end-of-life care, strong clinical judgment and the ability to work independently in the field, and excellent communication skills across care teams and families.
