BayCare Health System is seeking an experienced Case Manager to join our compassionate team in Pasco County. This is a field-based role that travels across Pasco County to provide behavioral health case management services for adults and children. The Case Manager will engage, assess, plan, coordinate, and monitor care, working with community partners to ensure continuity of care.
