Certified Behavioral Health Case Manager (TCM)

Behavioral Aid Solutions (BAS) is Mental Health Centers with two facilities — in Doral and Sunset-Kendall — serving Miami-Dade County and, via Telehealth, families across Florida. Our team of 70+ licensed professionals provides counseling and psychotherapy, family and couples therapy, infant/toddler and child/adolescent mental health services, psychological testing, psychosocial rehabilitation, and targeted case management. We are seeking a Certified Behavioral Health Case Manager (CBHCM or CBHCM-P) to provide Mental Health Targeted Case Management (TCM) services for children and/or adults within the Medicaid population. Case managers are often a family's most consistent point of contact — helping them navigate care, school, and community systems. POSITION RESPONSIBILITIES: Conduct needs assessments and develop individualized case management/service plans. Coordinate client access to community resources, medical/psychiatric care, school services, and social services. Provide case management services in the home, school, and community. Monitor client progress toward goals and adjust service plans as needed. Maintain accurate, Medicaid-compliant documentation of every case management encounter. Collaborate with treating clinicians, families/guardians, schools, and community agencies. QUALIFICATIONS: Education Bachelor's degree (required). Licenses / Certifications Active Florida Board Certification in Case Management (CBHCM or CBHCM-P), issued by the Florida Certification Board (FCB) (required). Experience: Minimum 1 year of experience providing direct behavioral health case management services to adults or children (preferred). Familiarity with Medicaid documentation requirements. Additional Requirements: Required to complete an AHCA Level II Background Screening, as mandated under Chapter 435, Florida Statutes. Reliable transportation for home, school, and community-based visits. Reliable, self-directed, and interested in participating in trainings within the company. REPORTS TO: Reports to the Targeted Case Manager Supervisor, and ultimately the Clinical Director. SCHEDULE: This is a flexible schedule; sessions and visits are scheduled directly between you and your assigned clients within program needs. Compensation is a competitive, productivity-based rate paid per completed unit of service.
