High Needs Case Manager Children Services Case Manager (Behavioral Health) Pay: Starting at $22.00 per hour

- Coordinate care and treatment activities for assigned clients.
- Participate in client staffing and implement individualized treatment plans.
- Provide supportive counseling and document services using the DIAP format.
- Complete assessments and reassessments under professional supervision.
- Develop and update treatment plans in accordance with documentation standards.
- Coordinate services with physicians, healthcare providers, community agencies, and support systems.
- Assist clients in navigating behavioral health and physical healthcare services.
- Maintain accurate, timely, and compliant client records.
- Participate in utilization review and performance improvement activities.
- Attend team meetings, trainings, and professional development opportunities.
- Maintain clinical productivity standards and quality care expectations.
- Promote culturally competent, client-centered care.
- Provide services to individuals identified as High Needs, including those designated as Seriously Mentally Ill (SMI).
- Conduct regular community and home visits.
- Assist clients during hospital admissions, discharges, and transitions of care.
- Coordinate Child and Family Team (CFT) and Adult Recovery Team (ART) meetings.
- Collaborate with DCS, DDD, probation/parole, schools, healthcare providers, and community resources.
- Complete referrals, prior authorizations, and required health plan documentation.
Candidates must qualify as a Behavioral Health Technician (BHT) and meet one of the following:
- High school diploma or GED and at least 2 years of behavioral health experience; OR
- Bachelor's degree in any field and at least 1 year of behavioral health experience; OR
- Bachelor's degree in a human services-related field; OR
- Completion of a Behavioral Health Technician training program; OR
- Master's degree in a human services-related field and not independently licensed.
- One year of behavioral health experience.
- Experience working with individuals with Serious Mental Illness (SMI).
- Case management or care coordination experience.
- Knowledge of community resources and healthcare systems.
- Understanding of behavioral health treatment and recovery principles.
- Knowledge of DSM-5 diagnostic criteria.
- Ability to coordinate physical and behavioral healthcare services.
- Strong documentation and organizational skills.
- Crisis intervention and problem-solving abilities.
- Effective communication and relationship-building skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Commitment to culturally responsive and trauma-informed care.
- Valid Arizona Driver's License.
- Acceptable driving record.
- Ability to travel throughout the community as needed.
- Ability to work flexible hours when required.
