Spectrum Healthcare is seeking a compassionate and dedicated Case Manager to support individuals with behavioral health needs by coordinating care, promoting wellness, and helping clients successfully navigate healthcare and community support systems.

As a Case Manager, you will play a vital role in improving client outcomes through care coordination, treatment planning, supportive counseling, and collaboration with healthcare providers and community partners. What You'll Do

• Coordinate care and treatment activities for assigned clients.
• Participate in client staffing and implement individualized treatment plans.
• Provide supportive counseling and document services using the DIAP format.
• Complete assessments and reassessments under professional supervision.
• Develop and update treatment plans in accordance with documentation standards.
• Coordinate services with physicians, healthcare providers, community agencies, and support systems.
• Assist clients in navigating behavioral health and physical healthcare services.
• Maintain accurate, timely, and compliant client records.
• Participate in utilization review and performance improvement activities.
• Attend team meetings, trainings, and professional development opportunities.
• Maintain clinical productivity standards and quality care expectations.
• Promote culturally competent, client-centered care.

High Needs Case Manager Responsibilities

• Provide services to individuals identified as High Needs, including those designated as Seriously Mentally Ill (SMI).
• Conduct regular community and home visits.
• Assist clients during hospital admissions, discharges, and transitions of care.
• Coordinate Child and Family Team (CFT) and Adult Recovery Team (ART) meetings.
• Collaborate with DCS, DDD, probation/parole, schools, healthcare providers, and community resources.
• Complete referrals, prior authorizations, and required health plan documentation.

Qualifications


Candidates must qualify as a Behavioral Health Technician (BHT) and meet one of the following:

• High school diploma or GED and at least 2 years of behavioral health experience; OR
• Bachelor's degree in any field and at least 1 year of behavioral health experience; OR
• Bachelor's degree in a human services-related field; OR
• Completion of a Behavioral Health Technician training program; OR
• Master's degree in a human services-related field and not independently licensed.

Preferred Qualifications

• One year of behavioral health experience.
• Experience working with individuals with Serious Mental Illness (SMI).
• Case management or care coordination experience.
• Knowledge of community resources and healthcare systems.

Skills & Knowledge

• Understanding of behavioral health treatment and recovery principles.
• Knowledge of DSM-5 diagnostic criteria.
• Ability to coordinate physical and behavioral healthcare services.
• Strong documentation and organizational skills.
• Crisis intervention and problem-solving abilities.
• Effective communication and relationship-building skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Commitment to culturally responsive and trauma-informed care.

Requirements

• Valid Arizona Driver's License.
• Acceptable driving record.
• Ability to travel throughout the community as needed.
• Ability to work flexible hours when required.
