Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews and observation, utilizing medical records to understand the emotional and social factors affecting recovery.

• Deliver individual and group counseling to patients and their families, while proactively addressing emotional adjustment issues, grief, and coping strategies.

• Collaborate closely with the interdisciplinary rehabilitation team to create and implement individualized treatment plans that encompass patients' psychosocial needs.

• Guide patients in accessing essential community resources, such as support groups, vocational rehabilitation services, and housing assistance, to aid their transitions back into the community.

• Educate patients and their families on available resources and aftercare services to ensure well-rounded support post-discharge.
\n
• Actively participate in discharge planning, facilitating seamless transitions to home or other care settings, ensuring continuity of care.

Patient Population / Client Focus\n

The patients you will serve are individuals recovering from a wide range of medical diagnoses, including but not limited to strokes, spinal cord injuries, orthopedic surgeries, and chronic illnesses. Your role will be instrumental in helping them cope with the emotional challenges that accompany significant health changes, ultimately supporting their journey towards achieving optimal independence.

Qualifications\n

• A Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
