Physical Therapist - Home Health

This position has a schedule of 8:00 AM - 4:30 PM.

We are seeking a dedicated Physical Therapist for a home health position serving the zip codes 19141, 19126, and 19120. This role involves evaluating and treating patients in their homes, ensuring they receive the highest standard of care.

Responsibilities:

• Conduct patient evaluations and develop individualized treatment plans.
• Implement therapy interventions and monitor patient progress.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Educate patients and their families on at-home exercises and care techniques.
• Document patient care and treatment in compliance with organizational standards.

Required Experience / Certifications / Licensure:

• Active Physical Therapist license.
• Prior experience in a home health setting preferred.
• Strong communication and interpersonal skills.
