Key Responsibilities\n

• Conduct comprehensive assessments for new and existing clients to evaluate their mental health needs and develop tailored treatment plans.

• Facilitate ongoing communication between supervisors, family members, and other healthcare professionals concerning client progress.

• Coordinate and schedule home visits as necessary, ensuring compliance with organizational policies and professional standards.

• Monitor and review client care through regular observations, chart reviews, and data analysis to ensure the effectiveness of intervention strategies.

• Participate in interdisciplinary team meetings, providing insights and updates on client progress to foster a collaborative treatment approach.

• Document monthly and quarterly progress notes that accurately reflect the client's development and any necessary adjustments to their treatment plan.

• Lead training sessions to empower staff in the implementation of program plans and efficient documentation practices.
