Registered Nurse (RN) - Case Management

Nurse Case Manager – Outpatient/Inpatient

Schedule: Monday to Friday, 8am-5pm & Callback on weekends/holidays

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: Normal

Requirements:

• Associate's or Bachelor's Degree in Nursing
• A valid, current, full and unrestricted Professional Nursing License to practice nursing in any U.S. state, Commonwealth of Puerto Rico, or U.S. territory
• Current Basic Life Support (BLS) certification by the American Heart Association
• Three (3) years of clinical nursing experience with one (1) year in an outpatient/inpatient setting
• Demonstrated knowledge of case management, contract health, and utilization review processes

Roles & Responsibilities:

• Serve as the Outpatient/Inpatient Nurse Case Manager, focusing on high-risk patients in specialty clinics.
• Meet health and social needs while ensuring quality, cost-effective healthcare.
• Facilitate care transitions from admission through discharge, ensuring continuity of care and post-hospital referrals.
• Collaborate with an interdisciplinary team to assess care plans and monitor patient progress.
• Evaluate patients for medical necessity and coordinate care for issues including abuse and chronic pain.
• Assist individuals with mental health issues in accessing necessary resources.

Compliance / Notes:

• Fingerprinting must be submitted and cleared prior to start.
• Shared contractor housing only; no pets or family allowed.
• Orientation every 2 weeks.
• COVID & FLU exemptions may be accepted upon submission of a declination form.
• Housing application must be submitted two weeks prior to start.
