Job Description

The Outpatient/Inpatient Nurse Case Manager serves as a supportive contact for high-risk patients, specifically focusing on specialty clinics requiring case management attention. This role encompasses meeting health and social needs while ensuring quality, cost-effective healthcare. Responsibilities include facilitating care transitions, ensuring continuity of care, and acting as a resource for nursing and other departments on transitional and regulatory issues.

Responsibilities

• Collaborate with an interdisciplinary team to assess care plans and monitor patient progress.
• Evaluate patients in the ED for medical necessity and care coordination needs.
• Assess potential admissions and determine appropriate admission status with clinicians/physicians.
• Propose and implement outpatient plans for safe discharge with physician approval.
• Coordinate care for patients dealing with abuse, chronic pain, and frequent utilization issues.
• Engage with a diverse range of patients to help them navigate available resources.

Required Experience / Certifications / Licensure

• Education: Associate's or Bachelor's Degree in Nursing.
• Licensure: A valid, current, full and unrestricted Professional Nursing License to practice nursing in any U.S. state or territory.
• Current Basic Life Support (BLS) certification by the American Heart Association.
• Experience: A minimum of three (3) years of clinical nursing experience with one (1) year in an Outpatient/Inpatient setting.
• Knowledge of case management, contract health, and utilization review processes.
