Key Responsibilities

• Conduct thorough clinical assessments to evaluate patients' mental, emotional, and social needs.
• Develop and implement individualized treatment plans based on evidence-based practices tailored to each patient's unique circumstances.
• Administer therapeutic interventions, including solution-focused therapy and cognitive-behavioral techniques, to assist patients in managing their mental health conditions.
• Document patient interactions and treatment progress meticulously to maintain accurate and timely clinical records.
• Collaborate with nurses, physicians, chaplains, and other healthcare professionals within the hospice multidisciplinary team, sharing insights that enhance patient care.
• Educate and support patients and their families about mental health issues, coping strategies, and available resources throughout the hospice experience.
• Continuously review current research and advancements in mental health and hospice care to enhance practice and patient outcomes.

Patient Population / Client Focus

In this role, you will engage with a diverse clientele that includes patients of varying ages facing terminal illnesses. Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of Virginia.

Preferred Experience
