LTSS Service Coordinator - Case Manager (Region A: Bay/Escambia/Santa Rosa)

Job Title LTSS Service Coordinator – Case Manager (Region A: Bay/Escambia/Santa Rosa) Schedule Monday-Friday 8am-5pm EST Location This is a field-based position. Candidates must reside in one of the following counties: Bay, Calhoun, Escambia, Franklin, Gadsden, Gulf, Holmes, Jackson, Leo, Liberty, Madison, Okaloosa, Santa Rosa, Taylor, Walkulla, Walton or Washington County. Alternate locations may be considered if candidates reside within a commuting distance from an office. Note: per our policy on hybrid/virtual work, candidates not within a reasonable commuting distance will not be considered unless an accommodation is required by law. Job Summary The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs. In collaboration with the person supported, the coordinator facilitates the Person Centered Planning process, including assessments, development of a comprehensive Person Centered Support Plan (PCSP), aligning with medical directors and interdisciplinary care teams, and managing the individual's physical health (PH), behavioral health (BH), and LTSS needs as required by applicable state law, contract, and federal requirements. How you will make an impact Responsibilities include: Perform face-to-face program assessments using pre-defined tools and motivational interviewing techniques; Coordinate and manage an individual's waiver (LTSS/IDD), BH, or PH needs, serving as the single point of contact with clinical healthcare management and interdisciplinary team; Manage non-clinical needs of members with chronic illnesses, co-morbidities, or disabilities to ensure efficient utilization of long-term services and supports; Document short and long-term service and support goals in collaboration with the member’s care team; Identify members who would benefit from an alternative level of service or other waiver programs; Serve as mentor or preceptor for new staff and assist in formal training and process improvement initiatives; Submit utilization/authorization requests with supporting documentation aligned with the individual's care plan; Report critical incidents to state and county agencies (Adult Protective Services, Law Enforcement); Participate in appeals, fair hearings, member grievances, and state audits. Minimum Requirements Bachelor's degree (BA/BS) and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience that provides an equivalent background as required by state law and contract; Preferred: BA/BS in a health care related field; Preferred: Bilingual in English/Spanish; Preferred: Experience in LTSS, case management, social work, or hospital discharge planning.
