LTSS Service Coordinator - Case Manager (Region C: Pasco/Pinellas)

Position Summary LTSS Service Coordinator – Case Manager (Region C: Pasco/Pinellas). Schedule: Monday‑Friday 8 a.m. – 5 p.m. EST. This is primarily a field‑based role; candidates must reside in Pasco or Pinellas County or be able to commute from a nearby county. Responsibilities Conduct face‑to‑face program assessments using predefined tools and motivational interviewing techniques. Coordinate and manage an individual’s waiver (LTSS/IDD, behavioral health, or physical health) as the single point of contact with the clinical healthcare management and interdisciplinary team. Document and facilitate person‑centered planning, including needs assessments and development of comprehensive Person Centered Support Plans (PCSP) and backup plans. Engage the member’s circle of support and oversee all physical, behavioral, and long‑term services in accordance with state law, contract, and federal requirements. Identify members who may benefit from alternative levels of service or other waiver programs and advocate for appropriate placements. Submit utilization/authorization requests with supporting documentation aligned to each member’s care plan. Report critical incidents to internal and external parties such as state and county agencies, including Adult Protective Services and Law Enforcement. Participate in appeals, fair hearings, grievances, and state audits. Mentor and pre‑train new staff; support process improvement initiatives as assigned. Minimum Requirements BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent combination of education and experience per state law and contract requirements. Preferred Skills, Capabilities and Experiences BA/BS degree in a health‑care related field. Bilingual in English and Spanish (strongly preferred).
