Key Responsibilities

• Conduct comprehensive evaluations for patients, reviewing their medical history and performing detailed assessments to guide treatment decisions.
• Clearly communicate evaluation findings and treatment plans to patients and their families, incorporating them into the goal-setting process to enhance engagement and compliance.
• Implement individualized treatment protocols based on evaluation results, adapting approaches as necessary according to the patient's progress and the evolving healthcare landscape.
• Collaborate with other healthcare professionals to ensure comprehensive care, documenting all relevant changes in patient condition and adjusting the plan of care as needed.
• Manage discharge planning and follow-up care effectively, documenting each step of the patient journey accurately and timely to provide continuity of care.
• Demonstrate effective time management skills, maintaining a full schedule while meeting productivity expectations and attending team meetings punctually.

Patient Population / Client Focus

As a Physical Therapist at Stay Graceful PT Clinic, you will work primarily with orthopedic patients, including those recovering from surgeries, injuries, and chronic conditions. Qualifications

• Current American Heart Association BLS for Healthcare Providers CPR certification is mandatory.
