Key Responsibilities

In your role as a Physical Therapist, your daily tasks will include:

• Designing and implementing individualized treatment plans focusing on enhancing mobility, managing pain, and preventing falls.
• Administering a range of therapeutic interventions including manual therapy techniques, therapeutic exercises, and customized balance training programs.
• Utilizing assistive devices and recommending mobility aids to facilitate residents' independence and safety.
• Collaborating with interdisciplinary teams to coordinate care and support seamless transitions between different caregiving services.
• Providing education to residents and their families about home exercise programs, mobility strategies, and additional safety measures to support ongoing recovery at home.
• Maintaining precise and timely documentation in accordance with the Skilled Nursing Facility's regulations and standards to ensure compliance and continuity of care.

Patient Population / Client Focus

Your primary focus will be on the geriatric population, many of whom may have complex medical histories including chronic conditions and disabilities that affect their functional abilities. You will work with residents recovering from surgeries, strokes, or injuries, providing targeted rehabilitation that aligns with their specific needs. This role requires sensitivity and compassion as you help residents achieve their rehabilitation goals.

Qualifications

To be considered for this position, candidates must possess:

• An active and valid Physical Therapy license in the state of Virginia.
• A Doctor of Physical Therapy (DPT) or a Bachelor's/Master's degree in Physical Therapy from an accredited program.

Preferred Experience

While two years of experience as a Physical Therapist is required, preference will be given to candidates with:

• Prior experience working in a Skilled Nursing Facility or geriatric rehabilitation setting.
