Key Responsibilities\n

• Serve as a clinician, working directly with clients both in the office and in the community to evaluate and develop treatment plans for individuals experiencing symptoms of major mental illness and severe emotional disorders.

• Conduct brief, evidence-based screenings and arrange in-depth assessments as necessary, ensuring a thorough understanding of each client’s unique circumstances.

• Collaborate with team members to create and implement integrated care plans, ensuring clients have access to necessary support services and resources.

• Provide a range of focused prevention, treatment, and recovery services, maintaining a client-centered focus that prioritizes individual goals and empowers clients to achieve meaningful outcomes.

• Assist in crisis intervention when needed, ready to provide immediate support and guidance to clients facing urgent situations.
\n
• Monitor ongoing case management by tracking client admissions, progress, and discharges, ensuring timely completion of necessary documentation and assessments.

• Actively participate in interprofessional team meetings to enhance service delivery and initiate quality improvement initiatives.

• Be available on-call during and after program hours as required to ensure client needs are met in a timely manner.

Compensation and Benefits\n

Heart Share Community Services offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
