Key Responsibilities\n

As a Licensed Clinical Social Worker in our hospice setting, your primary responsibilities will include:

• Providing individual and couples therapy sessions to clients of diverse age ranges and backgrounds, addressing emotional and psychological issues related to end-of-life care.

• Timely completion of clinical documentation, including session notes and treatment plans, ensuring compliance with regulatory standards and best practices.

• Managing your schedule by coordinating ongoing appointments and maintaining an organized calendar.

• Utilizing electronic medical record (EMR) systems to document and track client progress and treatment outcomes effectively.

• Collaborating with interdisciplinary team members to develop comprehensive care plans tailored to each client's unique clinical presentation.

Patient Population / Client Focus\n

The clients you will serve primarily consist of individuals and families coping with terminal illness and the accompanying emotional and psychological burdens. You will be required to demonstrate sensitivity, compassion, and skill in supporting clients through grief, loss, and existential concerns, ensuring that their voices are heard and respected throughout the process.

Qualifications\n

To be successful in this role, candidates must meet the following qualifications:

• A Master’s degree in Clinical Psychology or Social Work.

• Active and independent licensing as a LCSW in the state of Colorado.
