Care Coordinator

Supporting the Care Management department, the full-time remote Care Coordinator will manage outreach functions, case assignments, and member follow-ups while ensuring timely coordination of healthcare services. Key responsibilities Initiate outreach cycles and document member referral statuses within the electronic record Track and triage member needs, providing education and support for follow-up care and health management Communicate with external stakeholders to coordinate member care and assist with scheduling appointments and services Required qualifications High School diploma required; Associate's degree in a related field preferred 3-5 years of experience in a professional environment; healthcare experience is highly desirable Proficiency in Microsoft Office and ability to learn Casetrakker, JIVA, or MedHok within 3 months Strong knowledge of clinical services and healthcare operations Ability to work independently and as part of a team in a fast-paced environment
