Key Responsibilities\n

As a Licensed Professional Counselor, you will engage in various responsibilities, including but not limited to:

• Conducting thorough patient intakes that focus on diagnostic and functional evaluations, followed by formulating treatment recommendations for the primary care provider.

• Providing brief, focused interventions to individuals requiring mental health services and effective follow-ups to monitor patient progress.

• Working closely with primary care providers to manage co-occurring physical and mental health issues and recommending referrals to specialized mental health services when necessary.

• Developing relapse prevention plans for patients and assisting them in maintaining stable functionality.
\n
• Documenting patients’ progress, treatment plans, and recommendations in electronic health records to keep the care team informed.
\n
• Offering educational resources and self-management strategies to empower patients in their treatment journeys.
\n
• Consulting with clinic management and the health care team about behavioral health services, improving program outcomes and patient experiences.

• Adhering to the Code of Ethical Conduct for Clinical Professionals and performing additional duties as assigned to meet the needs of our patients and the clinic.

Qualifications\n

To be eligible for this position, candidates must meet the following requirements:

• A Master’s Degree in Clinical Counseling or a related field.

• Current LPC license is mandatory.
