Work Environment

This position is predominantly onsite, where you'll conduct home visits to provide therapy in a personalized setting. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, assessing their physical capabilities and limitations.
• Develop and implement tailored treatment programs that incorporate evidence-based practices to promote recovery and functional independence.
• Deliver high-quality therapeutic care to patients in their homes, employing techniques that directly address their needs and align with the physician's plan of care.
• Educate and coach patients and their caregivers on the rehabilitation process, ensuring they understand the goals and methods of treatment.
• Maintain accurate documentation of patient progress and treatment efficacy to meet clinical standards and support ongoing evaluations.
• Contribute to the overall effectiveness of the clinic by providing excellent customer service and fostering a welcoming atmosphere for patients and their families.

Qualifications

• A Bachelor's Degree from an APTA or CAPTE-accredited physical therapy program.

• A Basic Life Support (BLS) Certification from the American Heart Association.

• Proficiency in computer skills, including familiarity with electronic health records (EHR) and word processing software.

Compensation and Benefits
