Key Responsibilities\n

• Conduct comprehensive assessments by evaluating seniors' emotional, mental, and social needs, tailoring strategies to address their unique circumstances.

• Collaborate with clients and their families to create personalized care plans, setting specific goals in alignment with their needs and desires.

• Provide counseling and emotional support for seniors experiencing transitions, such as moving to assisted living or coping with loss.

• Advocate for clients’ rights and ensure access to necessary community resources, including healthcare services, counseling, and social programs.

• Engage with interdisciplinary teams, consisting of nurses, caregivers, and other healthcare providers, to enhance client care by discussing coordination of services during team meetings.

• Regularly visit clients, whether virtually or in-person, to monitor overall well-being, assess living conditions, and address any safety concerns.

• Facilitate open communication and conflict resolution among family members involved in the client's care, fostering a supportive family approach.

• Educate clients and their families about community resources, support groups, and programs designed to enhance social engagement, thereby improving their overall quality of life.

• Maintain meticulous documentation of client assessments, care plans, and progress notes, ensuring alignment with ethical standards and compliance with state regulations.

Qualifications\n

Compensation and Benefits\n

The position offers a competitive salary range of $60,000 to $100,000 annually, commensurate with experience.
