The ideal candidate has strong clinical judgement, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of skilled nursing care in the home setting. What You’ll Do Conduct OASIS assessments and complete state-of-care, recertification, resumption-of-care, and discharge visits Develop and implement individualized plans of care in collaboration with physicians and the interdisciplinary team Perform comprehensive nursing assessments and ongoing patient evaluations Administer medications, perform skilled nursing procedures, and provide wound care as ordered Educate patients and caregivers on disease management, medications, safety precautions, and infection prevention Supervise and coordinate care provided by LPNs, HHAs, and other team members as appropriate Monitor patient progress, update care plans, and maintain thorough, timely, and compliant documentation Ensure compliance with all Medicare, CMS, and home health regulations What We’re Looking For Current Florida Registered Nurse (RN) license (required) Previous home health experience is strongly preferred but not required OASIS experience preferred Excellent assessment, communication, time management, and documentation skills A compassionate, dependable, and professional attitude A team-oriented mindset and a genuine passion for making a difference in patients’ lives What Sets Us Apart A true culture of teamwork and family: we support one another and celebrate each success together A patient-first approach that respects patient preferences and promotes dignity Work-life balance with flexible scheduling, so you can care for your patients and your life outside of work A reputation as the “best in home healthcare” in our area, built on trust, quality of care, and long-standing relationships with patients and families How to Apply Please submit your resume and a brief introduction highlighting your experience and why you’re excited to join our team.
