Key Responsibilities\n

• Conduct virtual, comprehensive psychosocial assessments to evaluate clients' strengths, needs, and mental health status.

• Develop personalized treatment plans utilizing evidence-based practices, ensuring that interventions are tailored to meet each client’s specific challenges.

• Facilitate both individual and group therapy sessions through online platforms, employing a variety of therapeutic techniques to engage and support clients.

• Collaborate effectively with a multidisciplinary team, including nurses, physicians, and other healthcare professionals, to ensure seamless coordination of care.

• Advocate vigorously for clients' rights, assisting them in accessing essential community resources and support services that enhance their quality of life.

• Meticulously document client interactions, treatment progress, and outcomes in electronic health records to maintain accurate and accessible client information.

• Participate actively in team meetings, case conferences, and ongoing education to refine your clinical skills and stay abreast of advancements in mental health care.

Patient Population / Client Focus\n

Your primary focus will be on individuals experiencing terminal illnesses, their families, and caregivers. This role requires sensitivity and skill in addressing complex emotional and psychosocial issues, including grief, loss, and anticipatory mourning. You will approach each client with a deep commitment to fostering trust and rapport, understanding that many are navigating one of life’s most challenging transitions.

Qualifications\n

• Minimum of a Bachelor’s degree in Social Work from an accredited institution; a Master’s degree is strongly preferred.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.

• At least two (2) years of post-licensure experience providing clinical social work services, demonstrating a proven track record in therapeutic treatment.

• Ability to conduct comprehensive diagnostic assessments and develop effective treatment plans based on client needs.

• Excellent interpersonal and communication skills, with an emphasis on empathy and cultural competence to work with diverse populations.

• Strong commitment to ethical practices and social justice in mental health care.

Preferred Experience\n

• Experience working in hospice or palliative care settings.

• Familiarity with electronic health record systems and documentation standards.

• Certification(s) in evidence-based practices such as Dialectical Behavior Therapy (DBT) or Acceptance and Commitment Therapy (ACT) is a plus.

Compensation and Benefits\n
