Key Responsibilities\n

• Deliver individual and group counseling sessions based on personalized treatment plans, addressing the distinct needs of each patient and their families.

• Conduct thorough assessments and provide comprehensive case management services, ensuring timely referrals to additional support services as required.

• Utilize evidence-based treatment modalities, such as cognitive behavioral therapy, relaxation techniques, and motivational interviewing, to support patients in managing their emotional well-being.

• Participate in multidisciplinary team meetings to discuss patient progress, share insights, and coordinate care with medical professionals and other team members.

• Create and implement outreach programs focused on wellness initiatives, enhancing community awareness surrounding mental health and hospice services.

• Maintain detailed documentation of patient interactions, treatment progress, and any significant changes in psychological health to comply with organizational policies and regulatory requirements.

The target population for this role includes patients receiving hospice care, typically individuals facing terminal illnesses or life-limiting conditions. Qualifications\n

• Master’s degree in Social Work from an accredited program.

• Current and active Pennsylvania LCSW license.

Compensation and Benefits\n
