Key Responsibilities\n

• Conduct thorough evaluations and assessments of patients' needs and strengths, utilizing strong diagnostic skills to understand the emotional and behavioral factors affecting individuals and families.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring that strategies align with patient needs and family goals.

• Monitor patient outcomes and adjust care plans accordingly, working collaboratively with medical staff and family members to provide comprehensive support.

• Provide ongoing emotional and practical support to families, helping them navigate the complexities of hospice care and end-of-life decisions.

• Adhere to all federal and state regulations, as well as organizational policies, to maintain the highest standards of care and compliance.

• Build rapport with patients and families, utilizing a coaching model and natural environment teaching techniques to promote therapeutic relationships.

• Utilize evidence-based practices to enhance child and family development and well-being, ensuring a respectful and supportive approach to care.

• Facilitate communication and cooperation among team members, including attending team meetings and sharing insights on patient care.

Patient Population / Client Focus\n

In this role, you will predominantly support patients in hospice care, many of whom are facing serious and terminal conditions. Your focus will include providing emotional support for patients and their families, educating them about available resources, and facilitating access to appropriate services. Special attention will be given to ensuring that each family's unique needs are met with dignity and compassion.

Qualifications\n

• A Master's Degree in Social Work or a Doctorate of Social Work is required.

• A valid LCSW credential and licensure from the Department of Public Health is mandatory.
